Most IV in Cabo is sold as wellness — pretty packaging, beach menus, glossy add-ons. That model works fine for a healthy adult who wants a hydration top-up after a long pool day. It does not work when you are actually sick. Here are the signs you should bypass the wellness menu and ask for a medical IV at a real clinic.

1. You are running a fever

A fever above 100.4°F (38°C) means your body is fighting something. A wellness IV will not treat the cause. A medical IV at a doctor-supervised clinic includes a physician exam, a possible lab test to identify the infection, and prescription medications (antibiotics, antivirals, antipyretics) if appropriate — in addition to the rehydration.

2. You can’t keep fluids or food down

Persistent vomiting means your usual oral hydration route is closed and you may be developing electrolyte derangement. You need an IV with prescription anti-nausea (Zofran), often a Toradol for cramping, and a doctor to decide whether you need labs or an ER referral. A spa drip cannot legally include Zofran in Mexico.

3. Your headache feels different than your usual headaches

Sudden, worst-ever, with vision changes, weakness, slurred speech, or stiff neck — that is not migraine, that is a medical emergency. Skip the IV entirely and go to a hospital ER (we coordinate emergency transfers). Even an ordinary migraine deserves a medical IV (which can include Toradol and magnesium) rather than a wellness drip.

4. You were just at the ER or hospital

Post-discharge IV at your hotel is a clinical service, not wellness. The physician needs to review your discharge paperwork, coordinate with the hospital, and run the right fluids and meds — not whatever was popular on a beach-menu Tuesday.

5. You are on prescription medications, especially for the heart, kidney, or mental health

Blood thinners, lithium, diuretics, MAOIs, and stimulants all change which IV formula is safe. A spa operator without a physician on intake cannot make those calls.

6. You are pregnant — or might be

Pregnancy excludes several standard “wellness” ingredients (NAD+, high-dose vitamin C, certain herbal blends). Only a physician should select a pregnancy-safe drip and only after confirming a clinical indication.

7. You are over 65 or have a chronic condition

Heart failure, diabetes, CKD, COPD, autoimmune disease — none of these rule out IV therapy, but they all require formula adjustments that a non-medical operator is not qualified to make.

8. The dehydration is severe

If you are dizzy on standing, your skin tents when pinched, you have not urinated in 8+ hours, or your heart is racing — that is moderate-to-severe dehydration that warrants a physician-supervised IV and possibly labs. See our deeper guide on dehydration in Cabo.

9. You suspect food poisoning

“Montezuma’s revenge” can be a simple traveler’s diarrhea or it can be a bacterial gastro that needs antibiotics. A medical IV provider can prescribe and infuse on the same visit. A wellness operator can’t. Read more in our food poisoning guide.

10. You are tracking a documented diagnosis or planning to claim insurance

Travel insurance reimburses a clinically indicated IV when the visit is documented properly — H&P, diagnosis code, itemized invoice. A wellness operator usually doesn’t produce that paperwork; a medical clinic does it by default.

To be fair: if you are a healthy adult with no medications, no symptoms beyond ordinary tiredness, and you want a hydration top-up before a dinner, most reputable wellness providers will serve you safely. The line we are drawing is not snobbery; it is patient safety when any of the signs above apply.
